Listed are forms needed to obtain copies of fire or medical reports. There is no charge to obtain a copy of a fire report. Please be aware that there is a $5 charge for each copy of a medical report. Please note, medical reports will only be released to the patient, or the patient's guardian if the patient is a minor. Access the following forms to begin the process:
